To register at the surgery it is desirable if you could bring in your current NHS Number, please click the link Avenham Surgery - NHS
Paper forms are still available at our reception desk.
Once you have completed the registration forms an appointment will be offered to see the Health Care Assistant for a routine health check.
At the time of registering you will be nominated a GP, This will not stop you from seeing the other clinicians and you will always be offered an appointment where possible with your named GP or other doctors if requested.
